I might be late to show this video lecture by Randy Pausch, but I think it might be inspirational enough to go in the Really Must See category.

Randy is a Professor at Carnegie Mellon University, who was diagnosed with metastatic pancreatic cancer and given 3 to 6 months to live in August of '07.

This is his reprised "Last Public Lecture," titled "Really Achieving Your Childhood Dreams, reprised here on Oprah.
